Appointment Information

Thank you for scheduling your eye care with us. We look forward to working with you. There are a few things we would like to make you aware of to facilitate your appointment with us.

• If you would like to fill out our entrance forms before your appointment, feel free to print them and bring them along with you. If not, we will have them for you when you arrive.
• Please be prepared for dilation at your annual eye exam. It is the most important part of the eye exam.
• Please arrive 10 minutes before your appointment time. We strive to respect everyone's time and run our clinic in a timely manner. Unfortunately, if you are late for your appointment there is a high likelihood that you will need to reschedule your appointment.
• If your insurance co-pay is less than five dollars you may want to bring that with you in cash. It is a five-dollar minimum to use a credit card. We will also need your major medical card and photo ID at check-in.
• If you have had your eyes cared for by another eye care provider for an ongoing condition or surgery, please let us know in advance so we can attempt to get those records before your exam.
